LOCKOUT/TAGOUT:  29 CFR 1910.147

As defined by OSHA, “Lockout/Tagout” refers to specific practices and procedures utilized to protect workers from the unexpected startup or release of hazardous energy from equipment during maintenance or service activities. Violations of the Lockout/Tagout program are the current Number 1 citation issued by OSHA.

Does your facility have machinery or other equipment that has any source of electrical, mechanical, hydraulic, pneumatic, chemical or thermal energy? If so, you are required to comply with OSHA’s Lockout/Tagout Standard, 29 CFR1910.147.

Does your facility have all of the following items as required by the standard?
 
____ Energy Control Program including energy control procedures, employee training and periodic inspections.
____ Written energy control procedures for all equipment and machinery that may require maintenance or servicing.
____ Each written procedure includes a specific statement of the intended use of the procedure.
____ Each written procedure includes steps for shutting down, isolating, blocking and securing machines or equipment to control the hazardous energy.
____ Each written procedure includes steps for the placement, removal and/or transfer of lockout devices or tagout devices and the responsibility for them.
____ Each written procedure includes the requirements for testing the machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and any other energy control measures.
____ Each written procedure includes the requirements for the release from lockout and tagout.
____ Documentation of training for facility personnel including all “Affected” and “Authorized” employees.
____ Knowledge that “affected” personnel are not “authorized” personnel who have been trained to perform the lockout/tagout and the required servicing or maintenance on each specific piece of equipment.
____ Have you identified all affected employees.
____ Have you identified all authorized employees.
____ Documentation of re-training for all authorized and affected employees whenever there is a change in job assignments, change in machinery, equipment or process that presents new hazards or new energy control procedures.
____ Approved Lockout devices that utilize a positive means such as a lock, either key or combination type, to hold an energy isolating device in the safe position and to prevent the energizing of a machine or piece of equipment.
____ Are you using an approved Tagout device or prominent wording device, which can be securely fastened to an energy isolating device in accordance with your written procedure, to indicate that the energy isolating device and the equipment being controlled may not be operated until the tagout out device is removed.

Each of the items listed above is required by OSHA’s Lockout/Tagout Standard, 29 CFR 1910.147. Documented practices and procedures necessary to control the release of hazardous energy to protect your employees during maintenance activities are not only required by law, but it is extremely important for the protection of your facility personnel.
